Job Summary: The Family Connections Consultant provides direct, home-based support services to families involved with the Department of Child Safety (DCS). Responsibilities: * Work directly with families involved with the Department of Child Safety to support child safety and family stability. * Manage an assigned caseload and ensure timely completion of required services, documentation, referrals, and follow-up activities. * Conduct home visits and community-based meetings to assess family needs, monitor progress, and provide supportive interventions. * Conduct assessments to help ensure a safe environment for children. * Provide trauma-informed, family-centered interventions and support related to parenting, mental health, domestic violence, substance use, and other identified family needs. * Meet with families to provide consultation, education, and skill development related to family functioning and safety. * Support services that strengthen family protective factors and enhance caregiver capacities. * Assist with service planning, linkages, and transition to ongoing supports, including helping families access financial, social, medical, educational, and employment resources. * Provide substance use education and linkage to treatment providers as needed. * Monitor, observe, and document interactions and behaviors between parents/caregivers and children during home visits, including modeling appropriate behavior and de-escalating conflict. * Work cooperatively with the Department of Child Safety (DCS) to support assessments, service planning, required reports, and meetings. * Develop caring, supportive, strength-based, culturally competent, and trauma-informed relationships with families and community partners. * Complete accurate, timely, and thorough documentation, including assessments, progress notes, service plans, incident reports, activity logs, statistical reports, and other required records in accordance with program, contractual, funding, and regulatory requirements. * Establish and maintain confidential client records and case files. * Accurately record and submit time worked in accordance with organizational policies and wage and hour requirements. * Participate in an on-call rotation and respond to client or program needs in accordance with program requirements. * Maintain professional representation of A New Leaf in all interactions with clients, staff, and community members. * Attend meetings, trainings, and special projects as requested by management. * Perform other reasonably related duties as assigned. Qualifications: Education: * Bachelor of Social Work or a related bachelor’s degree required. Experience: * Minimum of one (1) year of experience in the human services field. * Experience working with multi-problem families and complex family systems. * Knowledge of family dynamics and family-centered service approaches. * Knowledge of community resources and ability to assist families in accessing services. * Experience supporting families impacted by substance abuse, child abuse and neglect, trauma, sexual abuse, or related challenges. * Experience working with families involved in child welfare, family preservation, family reunification, or Department of Child Safety programs preferred. Under the direction of the Director of Housing, Shelter & Support Services, this position oversees program operations, supervises Program Supervisors, and ensures services are delivered in compliance with AHCCCS requirements, contractual obligations, and agency standards. Qualifications: Education: * Master's degree in Social Work, Counseling, Psychology, Human Services, Public Administration, Healthcare Administration, Business Administration, or a closely related field required. * Arizona Board of Behavioral Health Examiners (AZBBHE) associate or independent licensure (LAC, LPC, LMSW, LCSW, LAMFT, LMFT, or comparable license) preferred. * Bilingual (English/Spanish), preferred. Experience: * Minimum of three (3) years of progressively responsible leadership experience in behavioral health, housing services, homelessness services, or a related human services environment required. * Minimum of two (2) years of direct supervisory experience leading professional staff and/or supervisors required. * Experience overseeing program operations, regulatory compliance, quality assurance, and performance management required. * Experience administering AHCCCS-funded behavioral health or housing stabilization programs preferred. * Experience working with individuals experiencing Serious Mental Illness (SMI), housing instability, homelessness, or other vulnerable populations preferred. * Experience in preparing reports, monitoring program performance, and supporting contractual compliance and audits preferred. Pay Rate: $24.00 per hour Schedule: Monday - Friday; 10:00am-6:00pm Flexibility for weekend availability to support clients and program Job Summary: The Program Supervisor provides day-to-day operational and staff leadership for the Faith House Emergency Shelter, ensuring safe, trauma-informed, participant-centered services are delivered in accordance with agency policies, licensing standards, contractual requirements, and best practices. This position supervises shelter staff, supports program operations, maintains compliance and documentation standards, responds to participant and facility needs, and partners with the Program Manager to promote a safe, effective, and supportive shelter environment for survivors of domestic violence. Responsibilities: * Model trauma-informed, participant-centered practices while maintaining professional boundaries and ethical standards. * Complete participant intake, orientation, crisis intervention, safety planning, interim service plans, and lethality/basic needs assessments. * Develop, implement, and update individualized FOCUS Plans in collaboration with participants. * Maintain complete, accurate, and timely participant documentation, case files, and electronic records in accordance with agency, funding, licensing, and confidentiality requirements. * Facilitate psychoeducational and support groups or provide oversight and coaching to staff facilitating groups to ensure services are culturally responsive and participant-centered. * Respond appropriately to participant crises, behavioral concerns, conflicts, and emergency situations using trauma-informed de-escalation techniques. * Coordinate participant referrals, resource connections, discharge planning, and transition services. * Provide day-to-day supervision, coaching, guidance, and technical support to assigned shelter staff. * Participate in recruitment, interviewing, hiring, onboarding, transfers, corrective action, separations, and employee development in partnership with the Program Manager and Human Resources. * Conduct program and facility orientation for new staff and ensure employees are trained on procedures, safety practices, confidentiality requirements, and professional standards. * Complete employee performance evaluations, competencies, and required documentation within established timeframes. * Conduct regular staff meetings and promote accountability, teamwork, and quality participant services. * Coordinate daily staffing and schedules to ensure appropriate coverage and continuity of shelter operations. * Review and approve employee timecards, leave requests, status changes, transfer requests, and other personnel transactions accurately and timely. * Monitor staff compliance with agency policies, licensing requirements, grant requirements, professional standards, mandatory training, and safety procedures. * Support the Program Manager with participant intake, discharge planning, utilization management, operational planning, staffing coordination, and continuous quality improvement initiatives. * Meet regularly with the Program Manager to review program operations, staffing needs, participant concerns, operational priorities, and problem-solving strategies. * Oversee the day-to-day operations, safety, security, cleanliness, and functionality of the shelter environment. * Complete and submit required reports, statistics, incident reports, daily activity logs, administrative documentation, and other required program records accurately and within established deadlines. * Review participant documentation to ensure accuracy, completeness, and compliance with agency, contractual, licensing, and funding requirements. * Support administrative reporting, contract compliance, audits, and quality assurance activities as assigned. * Maintain confidentiality of participant, employee, and organizational information in accordance with agency policy and applicable laws. * Participate in the shelter's on-call rotation and respond to participant, staffing, operational, facility, and emergency situations in accordance with agency policy. * Establish and maintain collaborative working relationships with participants, staff, community partners, law enforcement, healthcare providers, and other service organizations. * Participate in required meetings, trainings, committees, and professional development activities. * Perform other duties as assigned in support of program operations and organizational objectives.
