The WelbeHealth Transition of Care (TOC) Coordinator is responsible for assisting the Transitions of Care team in the provision of
safe and efficient delivery of medical services, as directed. Under the general supervision of medical staff, the TOC Coordinator
performs essential administrative and operational tasks that facilitate smooth transitions between care settings. This role works
collaboratively with members of the Interdisciplinary team. Including, but not limited to, primary care providers, nurse case
managers, social work, transportation, and participants to promote the WelbeHealth values, team culture and mission.

YOUR OPPORTUNITY We're hiring a Therapy Continuity Coordinator at Dreem Health, where you’ll play a central role in keeping patients engaged in therapy long after treatment begins. This is not a passive follow-up role. You will actively support the continuity journey for patients enrolled in our internal PAP treatment program. This is a high-impact role reporting directly to the Customer Success Manager, where you will contribute directly to the stability and growth of our recurring revenue model by keeping patients engaged in therapy through timely outreach, proactive retention efforts, and compassionate communication. WHAT YOU’LL DO * This is a non-clinical, operational role that partners closely with Sleep Technologists during the transition from treatment setup to long-term resupply. JOB SUMMARY: Sycamores provides a spectrum of equitable mental health services through various programs to multicultural populations of all acuities. Although it serves a variety of consumers, the agency focuses primarily on providing care to low-income, high-at- risk youth. The Purpose of the Transition Coordinator is to assist, coordinate and help implement our Philosophy within the transition process of our program participants. This position will also coordinate and coach staff on the most effective ways to engage and build relationships with program participants throughout transition stages within the designated TAY programs and with all TAY program participants. This includes assisting with TAY employment transitions, workforce curriculum, and workforce/transition development training for other CWS’s and program participants in response to the needs of the TAY programs. This also includes gathering outcome data related to success or barriers with the implementation and couching process of the transition phases within the program. Starting Pay Range: $23- $26 per hour (starting pay will be based on previous work experience and educational background.) JOB QUALIFICATIONS * Bachelor’s degree preferred. * At least 2 years of relevant experience preferred. * Maintains all required licenses and certifications. * As this position requires the driving of company vehicles as part of regular duties, driving is considered essential function of this position.
