Description Job Purpose: We are seeking a compassionate and collaborative Psychiatric Mental Health Nurse Practitioner (PMHNP) to
join our growing outpatient and tele-psych care team. In this role, you will work alongside physicians and other behavioral health
professionals to assess, diagnose, and treat individuals across the lifespan facing mental health challenges. You’ll play a key
role in expanding access to quality, person-centered care, helping clients take important steps toward healing and wellness.

85022 Yuma: 1960 E Palo Verde St., Suite 1 Yuma 85365- Part Time What You Will Do: Conduct comprehensive psychiatric evaluations
and assessments. Develop individualized treatment plans based on patient needs, diagnosis, and clinical presentation. Provide
psychiatric medication management and ongoing follow-up care. Monitor patient progress, treatment response, medication
effectiveness, and potential side effects. Collaborate with physicians, therapists, primary care providers, and other members of
the integrated care team. Provide care within Denova’s appointment structure, including 15-minute follow-up appointments and
45-minute psychiatric evaluations. Understand and work within Denova’s established Pay-for-Performance — P4P — compensation model.
Provide patient education regarding diagnoses, medication options, treatment expectations, and wellness goals. Maintain accurate,
timely, and compliant documentation in the electronic medical record. Support crisis intervention, safety planning, and
appropriate referrals when clinically indicated. Participate in care coordination to support whole-person health outcomes. Follow
all applicable clinical, regulatory, compliance, and organizational standards. Other duties as assigned. What We Need From You:
Active and unrestricted Arizona APRN/PMHNP licensure” if this is for Arizona. Active PMHNP certification required. Graduate-level
nursing preparation as required for PMHNP licensure and certification. Active DEA certificate, NPI, and required credentials for
provider enrollment and credentialing purposes. Must be willing to begin employment once credentialing is near completion.
Credentialing timeline may vary between 3–6 months, depending on provider status (new vs. existing). One to five years of clinical
experience preferred. Must be comfortable providing care to patients across the lifespan. Must have fluency in documenting an
electronic medical record. Must have active BLS certification. Strong project management skills, including complex projects
resulting in measurable program growth. Excellent communication skills Possess a valid fingerprint clearance card and have a
recent tuberculosis test on file. Superior analytical and problem-solving abilities are required. Your Work Schedule: This role is
designed as an onsite position with a hybrid care model, meaning providers will work onsite at their assigned clinic location

The DeKalb Regional Crisis Center provides crisis intervention and stabilization services for adults. The Center offers three
levels of care starting with the Crisis Services Center unit that receives walk-ins, transfers from emergency rooms, and
involuntary referrals for evaluation.
If further observation is needed, individuals are monitored in the Temporary Observation Unit for 12-24 hours.
Individuals needing a longer period of stabilization or substance withdrawal management (usually 5-7 days) are admitted to the
Crisis Stabilization Unit where ongoing care includes crisis counseling, medication, and assistance with discharge planning.
Position Summary:
The Advanced Practice Registered Nurse (APRN) in the Crisis Stabilization Unit (CSU) provides advanced psychiatric and medical
care to individuals in acute behavioral health crises. The APRN collaborates with a multidisciplinary team to evaluate, diagnose,
and manage mental health and co-occurring substance use conditions. This role includes medical oversight, psychopharmacological
management, crisis intervention, and coordination of care from admission to discharge in a fast-paced, trauma-informed
environment. This is a part-time 12 hour shift position providing coverage for a caseload of up to 18 patients on Saturdays and
Sundays every other weekend.
Key Responsibilities:
Clinical Assessment and Evaluation:
* Perform comprehensive psychiatric assessments including history, mental status exams, suicide risk assessments, and
differential diagnosis.
* Develop and implement individualized treatment plans in collaboration with the client and treatment team.
* Conduct medical screenings and physical exams to determine medical stability for CSU admission.
* Order and interpret diagnostic tests and labs; initiate medical treatment or refer when necessary.
Psychiatric Treatment and Medication Management:
* Prescribe and manage psychotropic medications according to best practices and agency protocols.
* Monitor client response to medication and adjust regimens as needed.
* Provide psychoeducation to clients and families about diagnosis, medications, side effects, and self-management strategies.
* Document clinical interventions, progress notes, and treatment plans in the electronic health record (EHR) accurately and
timely.
Crisis Intervention and Management:
* Provide immediate psychiatric support in behavioral health emergencies within the CSU.
* Participate in and support de-escalation efforts, including serving as a clinical lead during crises.
* Collaborate with the BERT (Behavioral Emergency Response Team) and other clinical staff to ensure safe and therapeutic
outcomes.
* Assess risk factors and participate in safety planning, discharge planning, and referrals for continuing care.
Interdisciplinary Collaboration:
* Serve as a clinical leader in interdisciplinary treatment team meetings.
* Collaborate with psychiatrists, nurses, case managers, peer specialists, and support staff to coordinate and optimize care.
* Communicate clearly with family members, external providers, and agencies to ensure continuity of care and community
reintegration.
* Consult with the Medical Director and/or supervising physician as needed, in accordance with scope of practice and
collaborative agreement.
Compliance and Professional Practice:
* Maintain licensure and certifications required to practice as an APRN in the State of Georgia.
* Adhere to legal, ethical, and regulatory standards including HIPAA, OSHA, DBHDD regulations, and agency policies.
* Participate in internal audits, quality assurance, and performance improvement initiatives.
* Engage in continuing education and professional development activities relevant to psychiatric practice and crisis
stabilization.

Experience:
* Minimum two (2) years of clinical experience in a psychiatric setting; crisis or emergency psychiatric care preferred.
* Experience working with individuals with serious and persistent mental illness (SPMI) and co-occurring disorders strongly
preferred.
Skills and Competencies:
* Clinical expertise in psychiatric assessment, psychopharmacology, and crisis stabilization.
* Strong communication, clinical documentation, and organizational skills.
* Ability to work effectively in a fast-paced, high-acuity setting.
