Overview: This role is generally responsible for assisting physician leadership with patient care and serves as a patient advocate while facilitating a cooperative and collaborative environment among all providers. The Clinic Nurse will work with other nursing staff to assure quality service for patients and will always act within scope of practice and under the authority of the physician leadership. This role will perform all responsibilities and duties within the limits established by the physician staff and consistent with the statutes of the Massachusetts Board of Nursing. This role will report to the Lead Clinic Nurse for clinical matters and to the Office Manager for operational matters. The role is non-exempt. Responsibilities: Responsibilities in clinic, in regards to direct patient care, will include, but are not limited to: * Oversee patient experience while patient in clinic. * Advocate for optimal patient care; make recommendations as appropriate. * Provide patient and caregivers with education on patient’s care, treatment and future prevention. * Administer wound care per MD/NP orders. * Ensure medical records are comprehensive, accurate and follow HIPAA guidelines. * Assess vital signs, use clinical judgement and nursing assessment. * Provide MD/NP with pertinent information needed for visit. Assure any diagnostic testing and/or medical records available for provider as needed. * Regulate flow of patients as they enter clinic until they are discharged. Ensure patient safety. * Assist patient with transfers using appropriate ergonomics and safety guidelines. * Communicate with patient’s care team; VNA, outside providers, supply companies, etc. * Order needed supplies for patient; wound care supplies, pneumatic pumps, stockings. Responsibilities for daily operations of clinic: * Ensure care team is prepared prior to patient arrival. Review chart, have appropriate supplies ready for MD/NP to provide optimal care/assessment. * Oversee appropriate infection control practices; handwashing, donning appropriate PPE, disinfecting of equipment and sterilizing tools between each patient per CDC recommendations. * Disinfect exam rooms between each patient. * Ensure exam rooms are stocked with necessary supplies. * Return phone calls in timely manner; to patients, providers, supply companies, etc. Complete necessary documentation of phone calls. * Ensure safety standards are maintained throughout the clinic. * Maintain patient logs; Tissue applications, procedure complications. * Ensure future surgical patients are prepared; diagnostic testing completed and in chart, education complete, Pre-Op notes complete for providers at least 72 hours from procedure. * Ensure timely follow up on any tests ordered; labs, wound cultures, biopsy results, radiology results. * Ensure MD/NP aware of results and that they are available for review. * Complete ad hoc projects as needed Requirements: * Minimum of ADN/LPN. BSN preferred. * Current license, in good standing, as RN/LPN/MA in Massachusetts. * Current BLS/CPR certification. * Wound care and vascular surgery experience preferred. * Able to work as member of team and work collaboratively with other clinicians. * Willing to be available for local and/or regional assignments as needed * Must feel optimal patient experience and customer service is important. The Regional Clinical Program Manager is responsible for the development, maintenance, implementation, and oversight of standardized, competency-based onboarding, training, professional development, and clinical readiness programs for Clinical Technologists (CTs) and Licensed Practical Nurses (LPNs) within clinic-based vascular service lines. This role serves as a clinical resource and dotted-line clinical leader to Clinical Technologist and LPN Leads within designated Markets, supporting workforce development, competency, clinical readiness, standardized workflows, and implementation of enterprise clinical initiatives. The ideal candidate will live in our new market within the New York and New Jersey areas. This is a HIGH TRAVEL position. JOB SUMMARY: The District Program Director (“DPD”) is responsible for the onsite, wound center (“Center”) leadership within RestorixHealth (“RXH” or “Company”) on an interim basis. The DPD represents RXH in an administrative role with regard to hospital administration, hospital departments, panel physicians, referring physicians and other RXH customers. The DPD is responsible for ensuring the delivery of consistent, high quality medical care as well as managing center operations including daily administration of the center, fiscal management, marketing, and community referral development, staffing and staff management, team building, budgeting, revenue cycle management, host-hospital interaction management and physician management in conjunction with the Regional/Program Director. PRIMARY RESPONSIBILITIES: * Provide and support mentorship for RXH designated roles (FDC, HBOT, staff nurse, PD) * Lead by example and model ideal behaviors while performing all duties and in interactions with staff and providers. * Work in collaboration with RXH/hospital Partner Human Resources Department to conduct interviews for prospective staff and determine suitability for hire. * Establish performance expectations, provide regular feedback, including input in performance appraisals, and consistently manage these expectations in conjunction with the Regional Director. * Coordinate or facilitate training and educational programs in collaboration with other support departments (facility Implementation Team, Revenue Cycle, Human Resources, and Clinical Operations) including but not limited to orientation of new staff, mandatory requirements, developmental needs, and competencies training. * Continuously assesses and coordinates the integration of the Center into hospital operations. * Services as a liaison between the hospital and the Company. * Utilizes interdependent team decision-making to ensure quality of care. * Manages resources that promote cost effective, efficient operation of the unit and positive patient outcomes. * Demonstrates critical thinking and independent decision-making utilizing policies and procedures, standards of care, and data analysis, e.g., manages crisis prevention and intervention at a unit level. * Provides written reports to hospital and Company as requested. * Administer and uphold all the Company’s values, policies, and procedures. * Continuously work towards the Company’s goal and vision * Performs other duties as assigned. PROGRAM OPERATIONS: * Participate in continuing education meetings, committees or workgroups and identify needs, both managerially and clinically, and support resources as needed. * Continually markets the Center to identify referral sources as identified in the strategic plan, both in-house and in hospitals defined geographic area. * Assures all policies and procedures are current and all manuals necessary to operate the Center are current to Company and hospital standards. * Be visible in wound clinic and HBO suite aligning supportive documentation for procedures (graft utilization, debridements) and alignment of DRG HBO components in conjunction with clinical staff. * Collaborate with the Medical Director and staff to develop, implement and manage RXH’s * Performance Improvement Plan to improve Quality Outcomes. * Support center with timely billing and utilization documentation requirements via EMR including registration, charge posting, and supporting medical documentation notes. * Ensure appropriate compliance with Clinical Practice Pathways and initiate, modify and effect unit-based changes in clinical practice to maximize patient outcomes. * Review retrospectively monthly outcome report(s) and collaborate with RXH/Hospital Partner to identify improvement opportunities. * Continuously assesses patient flow in the Center and hospital and implements improvements as appropriate. * Continuously assess revenue cycle and reimbursements for Center services and initiates processes necessary to assure the correct level of reimbursement for the hospital. * COMMUNICATION: * Facilitates communication between physicians providing care in the Center and between Center physicians and referral sources, and Center physicians and Company medical leadership. * Facilitates communication between Center staff, hospital, and Company. * Accurately and adequately maintains all required documentation to support patient care, billing, and other Center operations. * CLINICAL: * Overseas the day-to-day operations of the clinical team demonstrates the knowledge and skills necessary to provide the direction described in unit-based competencies, policies, and procedures. * Plans for appropriate patient populations and manages the plan of care with interdisciplinary team through weekly meetings with clinical team. * Participates in development of collaborative care with physicians and clinical managers. * Collects data to utilize and analyze outcome-based care management variance reports to evaluate patient progress. * Communicates with clinical manager and all team members regarding appropriate therapeutic interventions for patient/significant other/family. * Responsible for providing care at the center if necessary. * Delegates appropriate tasks to licensed staff, monitoring, and maintaining accountability for the quality of care. * Develops and coordinates the education plan appropriate to the patient/significant other/family learning style.
