Licensed Vocational Nurse (LVN)

This is a union eligible position.   JOB SUMMARY  HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.   Our Dual Diagnosis Detox facility at 815 Buena Vista Ave W (815) is a co-ed program with approximately 45 beds that provides clients with social model detoxification treatment for substance use disorder (SUD). Clients are supported by a team of medical, SUD and behavioral health professionals who provide counseling and education for substance abuse disorder, life skills, as well as individual and group therapy.   The Licensed Vocational Nurse (LVN) is an integral part of this interdisciplinary team that supports our clients in navigating evidence-based treatment plans and the healthcare system. Our goal is that our clients transition into the community with the skills and resources to sustain their recovery goals and optimal well-being independently. The LVN works with the Health and Wellness team in collaboration with the Addiction Medicine team in the provision of treatment with Medication Assisted Therapy (MAT) for clients with Opiate Use Disorder (OUD) and other substance use disorders (SUD). Provides support of treatment adherence via client education that assists them in developing strategies for taking their medication regularly and effectively. Coordinates care for the client with the residential behavioral health team, the addiction medicine team, primary care, and other internal and external healthcare resources for the client. Utilizes the Nursing Process, Harm Reduction, and Trauma Informed Care modalities in all actions taken to advocate for and support the client in their planning and goals.   KEY RESPONSIBILITIES  Leadership Responsibilities: * Works collaboratively with the Health and Wellness staff. * Provides clinical leadership for client health education, client medication support, and within the quality improvement infrastructure of the healthcare and residential program. * Participates in matters related to performance and quality improvement, planning, protocols, and goal setting. * Comfortable in changing systems, champions change, and lean process change. Organizational Responsibilities: * Proper and consistent documentation of required information accurately on Clinic records and reports.  Keeps up to date with operational and procedural requirements. * In conjunction with Director of Addiction Medicine, assures organizational readiness for accreditation surveys and ongoing monitoring and reporting of conformance to quality within the clinics. * Supports the interpretation of standards and processes to meet best practice standards established in the health services program.  * Coordinates and consults with the appropriate department heads within the agency to facilitate integration between programs.  * Provides direct care coordination support for clients to access all aspects of HR360 healthcare services. * Serves on appropriate committees and work groups and other related committees of agency. And perform other duties as assigned.    QUALIFICATIONS Education, Certification, and Experience * Active CA Vocational Nursing License. * BLS Certification must be obtained within 30 days of hire. * Experience and interest in working with safety-net populations and in treating substance use disorders. * Experience working successfully with issues of mental health, criminal background, and other potential barriers to economic self-sufficiency. Knowledge * Knowledge of electronic medical records. * Knowledge of co-occurring disorders and trauma informed treatment. * Knowledge of clinical documentation (treatment plans, progress notes etc.).

Description CLINIC LVN The Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for the elderly ) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, and promoting autonomy, quality of life and the ability of individuals to live in their communities. The Position : Under the supervision of the Registered Nurse (RN), the Licensed Vocational Nurse (LVN) provides nursing services to participants at the CEI day center. The LVN communicates effectively with all members of the day center team and other program staff to ensure that the participants are receiving care that is appropriate and follows the provider’s orders and the participants’ plan of care developed by the MDT. DUTIES AND RESPONSIBILITIES: Provides nursing services to participants as ordered by primary care providers in a timely manner within the scope of the ADHC. Administers, monitors and records prescribe medications and treatments, including medications, oxygen, etc. as ordered. Coordinates and implements appropriate health-screening programs for participants. When assigned, participates in MDT meetings and assists in the development and maintenance of the participant’s plan of care. Notifies the providers of any marked changes in participants’ condition, including discovery of any wounds, any acute physical or behavioral changes. In conjunction with RN initiates follow-up services and provides care coordination as needed. Provides teaching, counseling, and health education to the participants and family members regarding nursing care needs. Initiates preventative and rehabilitative nursing procedures as appropriate for the participants’ care and safety. Contacts consulting physicians for participants as necessary. Responsible for supervision of personal care services provided by center aides and completion of daily personal care flow sheets as delegated. In conjunction with RN, it assists with emergency response and arranges emergency services such as emergency transportation. Participates in department and agency meetings. Completes all required paperwork as assigned. Participates in all agencies' quality improvement programs (QI) as assigned. Maintains the confidentiality of all company procedures, results, and information about participants or families. Follow all CEI Policies and Procedures. Complies with all agency training requirements Maintains a safe working environment by following CEI’s safety P&P’s. Maintains a courteous, helpful, and professional attitude on the job. Displays a willingness and ability to be responsive to all customer groups Performs other duties as assigned in a positive and helpful manner. QUALIFICATIONS: Graduate of a school of professional nursing. Have a current California Vocational Nurse License. A minimum of one (1) year acute experience, and experience working with the frail or elderly population. Excellent telephone /customer services skills Exercises initiative and flexibility. Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff, and the public. Maintains eligibility for licensure by obtaining required continuing education units. Excellent interpersonal skills . Well organized Ability to effectively solve complex issues. Strong verbal and written communication skills Ability to work as a team player in a multi-cultural, multi-disciplinary setting.
