The Licensed Practical Nurse (LPN) is responsible for providing direct care to the residents and provide supervision over the day-to-day activities of the nursing staff and nursing aides in accordance with current federal and state standards, guidelines, and regulations that govern the Plymouth Harbor policies and procedures. SHIFT AVAILABILITY: Day Shift: 6:30pm- 7:00am Night Shift: 6:30am-7:00pm DUTIES/RESPONSIBILITIES 1. Assist in the planning, development, organization, implementation, evaluation, and direction of assigned shifts’ activities. 2. Meet with administration, medical and nursing staff, and related departments in planning department services, programs, and activities. 3. Assist in development, implementation, and maintenance of nursing objectives and standards. 4. Assist in development, implementation, and coordination of departmental policies and procedures, resident care plans, nursing procedure manuals, and job descriptions. 5. Ensure that departmental personnel, residents, and visitors follow established policies and procedures at all times. 6. Be involved in the planning and implementation of recommendations by the Care Plan Team, RISK Committee, Utilization Review Committee, and Medicare Stand Up meetings. 7. Ensure that documentation of residents’ daily medical record is accurate, comprehensive, and timely. 8. Receive, document, and implement physicians’ orders in an accurate and timely manner. 9. Coordinate admit and discharge of residents. 10. Ensure timely response to resident requests/call lights. 11. Coordinate Home Care services and Smith Care Center services as indicated. 12. Ensure the proper recording and accurate count of schedule drugs at all times. 13. Ensure security of locked areas; monitor that only authorized staff is in possession of keys. 14. Ensure accurate and comprehensive reporting of resident condition via shift-to-shift reporting. 15. Interpret departmental policies and procedures to personnel, residents, visitors, and family members. 16. Visit each resident at least once during a shift to ensure appropriate care is being rendered and act accordingly. 17. Ensure that Residents’ Bill of Rights is followed at all times. 18. Ensure that residents have a clean, safe, orderly, and comfortable environment. 19. Assist in the development of and participate in programs designed for in-service education. Monitor orientation for unlicensed new hires. Orient licensed new hires. 20. Establish and maintain rapport with the medical profession and other health related facilities and organizations. 21. Assist in identifying departmental problems and subsequent resolutions. 22. Make recommendations to the Director of Health Services regarding staffing, improvement of services, and potential problem areas. 23. Participate in discharge planning, development and implementation of resident care plans and resident assessments. 24. Perform administrative duties as required by departmental policies and procedures and report to the Director of Health Services. 25. Assist in coordinating transportation to other health care facilities and individuals as indicated. 26. Participate in departmental projects and studies as assigned. 27. Coordinate health services, i.e., admissions, discharges, etc. 28. Ensure that a stock level of medications, medical supplies, equipment, and other supplies is maintained. 29. Inspect nursing care service areas and practices regularly to ensure compliance with current applicable regulations. 30. Cooperate with resident’s physician in planning nursing care; periodically reviewing and updating care plans with MDS Coordinator. 31. Complete medical forms according to departmental policies and procedures. 32. Notify the resident’s attending/alternate physician, family, or guardian, of any change in condition, accident, or incident according to established facility policy. 33. In cases of death, implement arrangements according to resident’s, families, or guardian’s wishes and in compliance with facility policy. 34. Evaluate emergencies, determine emergency measures to be used, and notify the Director of Health Services of the emergency. 35. Assist in the standardization of methods in which work will be accomplished. 36. Collect laboratory specimens as ordered. 37. Assist in establishing the administration, procedures, and control of medications, etc., as recommended by the pharmacists/Pharmacy Committee. 38. Perform prescribed monitoring and diagnostic and therapeutic procedures. 39. Assist medical professionals, as needed, during on-site visits. 40. Follow Universal Precautions and Infection Control procedures. Adhere to isolation techniques when appropriate. 41. Comply with Florida State statutes regarding required continuing education credits. 42. Attend and participate in workshops, seminars, and mandatory in-services as approved or required. 43. Maintain work area, equipment, and supplies in a clean, sanitary, and organized manner. Ensure that work areas are neat and confidential materials are properly stored before leaving area on breaks, end of workday, etc. 44. Work safely; complying at all times with Plymouth Harbor safety standards. 45. Call 555 in the event of an emergency. 46. Discharge properly such other responsibilities and duties as the Director of Nursing, Assistant Director of Nursing, or Sr. VP of Health Services may direct. SUPERVISORY RESPONSIBILITY * The LPN has supervisory responsibility for Certified Nursing Assistants. QUALIFICATIONS * Graduate of an accredited school of licensed practical nursing. * Minimum one year in the field of Long-Term Care preferred. * Must have good organizational skills. * Must be a detail-oriented person and able to perform duties with great accuracy on a daily basis. * Must have excellent personal skills, demonstrating the ability to conduct oneself in a non-controversial style; a style that inspires the respect of others and promotes a sense of trust in the individual’s competence. * Must have good communication skills; speaking, writing, and listening. * Must be fluent in English. * Must have good computer skills with the ability to learn and use proprietary software as required. CERTIFICATES/LICENSES/REGISTRATIONS * Must have valid certification as a Licensed Practical Nurse in the State of Florida. * Must have a current CPR (Cardiopulmonary Resuscitation) certification. PHYSICAL REQUIREMENTS * Must possess fully functioning sense of sight/hearing or use prosthetics that will enable these senses to function adequately in order to do the job. * Must be able to sit, stand, walk, and move throughout the workday (lifting, carrying, bending, squatting, reaching, kneeling, pushing, pulling, twisting) using good body mechanics. * Must be able to lift/push up to 100 pounds (with assistance if lifting). * Must be able to cope with the physical and emotional demands of the position.
BASIC FUNCTION: The primary purpose of the position is to direct the day-to-day functions of the nursing unit and to plan, organize, develop and direct the overall operation of the Department in accordance with current Federal, State, and local standards, guidelines, and regulations to assure that the highest degree of quality care is provided to the residents at all times. Provide professional assistance to all residents and staff of St. Paul's Senior Homes and Services when not precluded by other duties. ESSENTIAL DUTIES AND RESPONSIBILITIES: * Assumes responsibility and accountability of directing the activities and programs of the facility, in consultation with the Administrator, COO, or CEO as necessary, in the absence of the Administrator. * Supervise Assistant Director of Nursing in daily responsibilities and duties per DON’s discretion. * Participate as a member of the “Culture Change” committees and teams, partnering with the Administrator to assist in developing and implementing culture change projects. This may include attendance at education seminars/meetings and instructing/education staff members regarding “Culture Change”. * Serves on various committees of the facility as directed or as may become necessary, or as required by Federal, State, or local regulations. Also serves as the facility’s Safety Officer, chairing the facility’s safety committee, and attending and representing the Health Center on the St. Paul’s Safety Officer’s Committee. * Evaluates and implements recommendations from the facility's committees and/or consultants. * Ensures the availability of a responsible staff member to assume administrative responsibility in the absence of the Director of Nursing. * Plans, develops, organizes implements, evaluates, and directs the Nursing Department, as well as its programs and activities, in accordance with current State and Federal regulations and guidelines that govern Nursing Facilities, maintaining regulatory compliance at all times. * Develops, maintains, and periodically updates written policies and procedures as necessary and at least annually that govern the day-to-day functions of the Nursing Department. * Develops and maintains written position descriptions and performance evaluations for each staff position. * Assists department coordinators (activities, social services, etc.) to establish a rapport in and between departments so that each may be an effective team member. * Interprets and implements the facility's policies and procedures to employees, residents, families, visitors, government agencies, etc. * Participates as a member of the St. Paul’s Weekend Manager Schedule. * Represents the facility in dealings with outside agencies. * Makes written and oral reports and recommendations to the Administrator concerning the operation of the facility and Nursing Department. * Participates in the development of a plan of correction for deficiencies noted during survey inspections in co-operation with the Administrator. * Coordinates the Annual Patient Care Policy Committee meeting. * Monitors the Infection Control, Safety and Quality Assurance programs of the Department. * Makes routine inspections of the facility to assure that established policies and procedures are being implemented and followed. * Assists the MDS Nurse., Charge Nurses and Director of Staff Development to ensure compliance with all Federal, State and Local regulations. Directs and supervises the Unit Clerk ensuring the appropriate cordial response to residents, visitors, physicians and staff and the employee’s productive work performance. * Generates creative and innovative ideas that will assist the Administrator to achieve the goals and objectives of St. Paul’s Senior Homes & Services. * Coordinates personnel development and staff mentoring. * Interacts and evaluates the effectiveness of contracted healthcare services, including PT, OT, ST, and diagnostic services. * Ensures that subordinates comply with personnel requirements for annual physicals, TB testing, and required education. * Maintains confidential staff files and ensures personnel records are complete for all subordinates. * Performs other duties as may be assigned by the Administrator, COO or CEO. MINIMUM QUALIFICATIONS: * Must be able to communicate with, be interested in, and have compassion for the aged. * Must have good communication skills, both verbal and written in the English language. * Must have excellent organizational skills to meet deadlines and goals. * Must possess the ability to make independent decisions when circumstances warrant such action. * Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. * Must be knowledgeable in nursing and medical practices and procedures, as well as the laws, regulations, and guidelines that pertain to skilled nursing care and personnel administration. * Must possess the ability to work harmoniously with, and supervise both professional and non-professional personnel. * Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures that are necessary for providing quality care and maintaining a socially, morally and fiscally responsible operation. * Must be able to cope with multiple responsibilities in a changing and evolving environment. EDUCATION AND/OR EXPERIENCE: * Graduate of an accredited school of nursing. * A minimum of 3 years management experience in a skilled nursing facility. * Recommended background would be five years as a Director of Nursing or a combination of three years as Director of Nursing and two years as Director of Staff Development. * Must possess a current license to practice as a Registered Nurse in the state of California, and must possess a current CPR card. PHYSICAL REQUIREMENTS: * Frequently must be able to push and pull objects up to 50 pounds. * Must be able to lift and carry objects more than 20 pounds and occasionally up to 50 pounds. The ability to perform difficult manipulative skills is expected and will occur occasionally too frequently. * Must be able to walk, stand, stoop, squat, bend, and stand and sit for prolonged periods of time, in some cases frequently. * Must be able to frequently identify objects at a distance of 50 feet, read fine print and discriminate among colors. * Hearing will frequently involve distinguishing normal sounds where there is some background noise. Consistently must be able to speak clearly and make self-understood.
Gilead is looking for an experienced Per Diem RN to fill day shift hours as need across several of our adult residential support programs in the Middletown area and Clinton area. The Program Nurse is responsible for ensuring and advocating for the well-being and stability of individuals through the support of non-medical staff members. The RN also oversees health-related medical care and pharmacological issues. Applicants must demonstrate competence in and knowledge of community treatment, psychopharmacology, and crisis intervention. Minimum Requirements: * Must be a Registered Nurse, preferably with a Bachelor’s degree from an accredited program. * Connecticut licensure for Registered Nurse required. * Five years of experience in the mental health and/or addictions field. * Must maintain and provide proof of valid driver’s license and automobile insurance in good standing. This is a non-union position and pays $40.35 - $49.00 per hour dependent on level of education and experience Responsibilities: * Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders as well as medical knowledge related to health and well-being * Supporting the Program Director in leading an effective program through clinical services and best practice models of care * Maintaining contact with MDs (i.e. psychiatrists, PCPs, specialists, etc.) to coordinate medical and psychiatric care of individuals * Working closely with pharmacies to facilitate proper pharmaceutical care * Providing guidance and peer-support regarding medical care in their respective program * Conducting Health Histories, Health Assessments, and medical evaluations of clients’ emergent medical issues * Making appropriate community referrals based on assessments and evaluations to meet clients’ basic health needs * Providing ongoing medical education to case managers informally and in training formats * Ensuring proper orientation, training, and management of the medication self-administration system * Completing all required documentation electronically and on paper in a timely manner * Acting with compassion, integrity, and ethics Gilead employees are held to standards and regulations set by the Department of Mental Health and Addiction Services (DMHAS) and other regulatory bodies.
