Title: Service Coordinator (Mental Health Case Manager)
Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas
necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the
interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the
Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit,
working closely with the nursing staff to ensure continuity of treatment efforts.

About the Campus: Located in the beautiful central California coastal city of Santa Barbara, clients are welcomed into our safe,
calming, and homelike environment, where they will receive compassionate recovery support from our diverse team of clinical staff
who are trained in Trauma-Informed Approaches of care. Our clients receive comprehensive mental health services and support for up
to 23 hours. After this, they are referred to mental health services in the community that will best meet their needs so that they
may continue their recovery journey.
The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to
the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience,
licensure, and program acuity.

Program: Anchor Inn City | 215-35 Northern Blvd, Bayside, NY 11361 What You’ll Do If you want to use your Case Management skills to help struggling and vulnerable families, this could be a great opportunity for you. Anchor Inn, located in Bayside, Queens, is an emergency shelter for asylum-seeking families with children. As a Case Manager, you’ll provide culturally sensitive, trauma-informed intake and assessment for your clients and track their progress in the CARES case management system. You’ll coordinate for and provide service linkages to medical and mental health care, immigration legal services, childcare, school registration and related services, ESL instruction, interpretation and translation services, and more. You’re a great fit for this role if you have: * Bachelor’s degree in social work or a related field, or equivalent skills and experience. * Bilingual fluency in a second language, ideally Spanish/English. * Case management experience working with vulnerable people who have experienced significant trauma. * Crisis intervention experience. * Basic understanding of immigration laws and procedures. * Knowledge of child development, child welfare, and education requirements. * Great organizational skills and the ability to learn quickly and problem-solve under pressure. * Strong computer skills required, with knowledge of CARES a plus.
