individuals with disabilities in 12 states. We are now hiring an Intake Coordinator in Columbia, SC . The Intake Coordinator is
responsible for the overall coordination and facilitation of program admissions. This person is responsible for the admission
processes of new individuals and all functions keeping existing individuals current and in good standing. Responsibilities Serve
as the point person for all admissions into programs Assure service authorizations in Therap are created and maintained to avoid
loss of services and payments Gather necessary documents for all new admissions Attend pre-placement meeting for all new
admissions to ensure all necessary documents are collected prior to an admission Enroll/admit all individuals into Therap Work
collaboratively with billing department and regional offices for troubleshooting issues impacting billing and cash flow Generate
billing data from Therap to billing specialist, when necessary Employee must cooperate with the license and department staff in
any inspection, inquiry, or investigation Minimum Requirements High school diploma or equivalent; Bachelor’s or associate degree
preferred Working Conditions Valid driver’s license with good driving record Knowledge and understanding of local, regional, and
state regulatory operations Experience with intake and admissions Familiarity with social security and Medicaid Working knowledge
of ICD-10 codes Therap experience is a plus Proficiency in Microsoft suites Employment is contingent upon successful completion of

Essential Functions: * Complete crisis and initial intake assessments with clients, scheduled and walk-ins. * Assess patients throughout their episode of care to determine medical necessity and to create an individualized plan of care for the identified diagnosis. * Provide crisis/linkage case management to ensure continuity of care. * Provide and coordinate a continuum of treatment services between the ED/SCH and BHAL referrals to meet patient needs. * Comply with productivity standards. * Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner. Position Qualifications: * Bachelor's degree in social services or related field required. * Master's degree preferred. * Background check clearance required. * LSW/LCSW/LCPC preferred. * Minimum of one (1) year experience working with adult clients with mental health diagnosis, conducting counseling, crisis intervention or case management required. * Proficient in the use of the EHR (Electronic Health Record) system required. * Documented proof of passing the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) training and test required. * Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing. Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
