This position leads clinical excellence across the full patient journey, from inpatient rehabilitation through to community-based transitional care, supporting complex care coordination, optimal functional outcomes and person-centred service delivery. Working closely with the Nurse Unit Manager, TACP Manager and WMH Executive, the CNC2 plays a key role in clinical service development, workforce capability building, and the advancement of evidence-based practice across both services. The role provides high-level clinical guidance to multidisciplinary teams, drives quality improvement initiatives, and contributes to strategic planning, research and education aligned with ARNA competencies and NSW Health frameworks.
