About the Role
Practice nursing with the autonomy, time, and patient connection that allows you to make a meaningful impact every day. As a Home
Health Registered Nurse, you'll deliver comprehensive skilled nursing care to patients in their homes, supporting recovery,
chronic disease management, and overall health goals while helping individuals remain safely in the comfort of their own
environment.
This role is ideal for an RN who values clinical independence, critical thinking, and building strong relationships with patients
and families over time. You'll manage a caseload of patients from admission through discharge, providing skilled nursing care,
conducting assessments, and developing individualized care plans tailored to each patient’s needs. Working closely with physicians
and an interdisciplinary care team, you'll coordinate services, make informed clinical decisions, and play a central role in
shaping patient outcomes and supporting each person’s journey toward greater health and independence.

This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes,
assisted living facilities, or nursing homes

KEY RESPONSIBILITIES The Administrator is responsible for the overall direction of the Home Health services. The Administrator is responsible for the employment of qualified Home Health personnel; is responsible for the provision of home health services, directly, and the delegation to and coordination of hospice personnel evaluations; is responsible for establishing standards of care to comply with federal and state regulations and guidelines. The Administrator establishes, implements, and evaluates goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization and philosophy. In this role you will be responsible for: * Provides expert guidance and assistance in all matters relating to personnel and patient care services. * Responsible for providing oversight and helping to manage patient and personnel assignments to ensure optimal management of the complex and interdisciplinary care of home health patients * Responsible for ensuring coordination of care for patients through development and implementation of policies and procedures, personnel education and in-services, and assisting in care plan development with clinical staff * Responsible for ensuring coordination of referrals for patients * Responsible for ensuring the assessment of patient needs and ensuring the plan of care is developed, implemented, and updated * Assists in the development and interpretation of the philosophy and objectives of the agency Required Qualifications * Bachelor's Degree in Nursing * Current state license as a Registered Nurse * Minimum 2 years of experience in nursing * Minimum 2 years of experience in home health * Proven experience with regulatory and/or leadership experience * Demonstrated knowledge of the appropriate skills for communicating with all ages, especially the geriatric population Location * Office Location: Carmel, IN Hours * 8:00 AM – 5:00 PM, Monday through Friday Pay Range $93,500—$104,000 USD How We Care for You * Minimum of 3 Weeks Paid Time Off (PTO) * Medical, Dental, and Vision Insurance * HSA and FSA options including Dependent Care * Company paid Short Term Disability * Company paid Life Insurance * 401(k) with Employer Match * Mileage Reimbursement * Company Vehicle Program for field roles * Pet Insurance * ID and Fraud Protection * And more...
