The WelbeHealth Transition of Care (TOC) Coordinator is responsible for assisting the Transitions of Care team in the provision of safe and efficient delivery of medical services, as directed. Under the general supervision of medical staff, the TOC Coordinator performs essential administrative and operational tasks that facilitate smooth transitions between care settings. This role works collaboratively with members of the Interdisciplinary team.
