Arbetsuppgifter Som sjuksköterska i vårdplaneringsteamet ansvarar du för en trygg och säker hemtagning av patienter från slutenvården samt uppföljning på korttidsvistelse. Du arbetar utifrån ett individcentrerat förhållningssätt med fokus på att skapa förutsättningar för den enskilde att leva ett så självständigt, delaktigt och meningsfullt liv som möjligt. Tillsammans med dina kollegor planerar du för hemgång från sjukhus genom digitala planeringar, personliga möten eller telefonkontakt. Arbetet innebär även att du gör bedömningar och överrapporteringar enligt Hälso- och sjukvårdslagen (HSL) till andra verksamheter i samband med hemgång från sjukhus och korttidsboende. Du samverkar dagligen med flera olika yrkesgrupper både internt och externt. En viktig del av arbetet är att skapa goda relationer med närstående samt samarbeta nära biståndshandläggare, arbetsterapeuter och fysioterapeuter för att ge brukare och närstående trygghet och god service.
