Du kommer att rotera mellan Fagersta kommuns hälso- och sjukvård och Region Västmanlands Närvårdsteam. Arbetsuppgifterna kommer bland annat bestå av att samordna, leda, delegera och ansvara för hälso- och sjukvårdsinsatser. Dessutom kommer du att planera genomföra och utvärdera omvårdnadsinsatser. För att lyckas i rollen och för att tillgodose våra patienters behov är samverkan såväl internt som externt mycket viktigt. Som sjuksköterska kommer du även att handleda, ge råd och stöd till övriga professioner i teamet. Du kommer att rotera mellan: Fagersta kommuns hälso- och sjukvård: Här kommer du att arbeta med patienter på särskilt boende, korttidsenheter, hemsjukvård samt inom LSS och psykiatri. Närvårdsteam Fagersta och avancerad hemsjukvård: Här kommer du att arbeta med patienter som är för sjuka för primärvård men för friska för sjukhusvård, och ge dem en individanpassad vård i hemmet. Kvalifikationer Vi söker dig som är legitimerad sjuksköterska. Du behöver ha erfarenhet av att arbeta som sjuksköterska, gärna med erfarenhet från kommunal hälso- och sjukvård och/eller avancerad hemsjukvård. Vårt arbete styrs av dokument som många gånger är på svenska och du behöver därför ha goda kunskaper i det svenska språket, både i tal och i skrift. Har du erfarenhet av arbete i journalsystemet Cosmic ser vi det som en fördel. Vi ser att du är en trygg, stabil person med god självinsikt. Du samarbetar bra med andra såväl som att du har en god förmåga att arbeta självständigt. Rollen som sjuksköterska kräver att du kan leda omvårdnadspersonal på ett motiverande sätt.
